By submitting this form, I acknowledge and agree to the following:

  1. Purpose of Service

    I understand that the free headstone cleaning service offered by Lufkin Headstone Cleaning is provided for the purpose of documenting the cleaning process and sharing the historical or personal story of the individual memorialized. The service is offered at no cost to me.


  2. Permission for Photography & Video

    I grant Lufkin Headstone Cleaning permission to photograph and/or record video of the headstone, surrounding area, and cleaning process. I also grant permission for this media to be used, edited, and shared publicly on websites, social media, and marketing materials associated with Lufkin Headstone Cleaning.


  3. Storytelling & Publication Rights

    I understand that the name, dates, and publicly visible inscriptions from the headstone may be included in posts or videos. If I choose to share additional personal or historical information about the deceased, I consent to that information being used as part of the storytelling content.


  4. No Financial Obligation

    I acknowledge that this cleaning is provided completely free of charge and that Lufkin Headstone Cleaning assumes no responsibility for damage that may result from normal cleaning processes performed in accordance with standard preservation practices.


  5. Authorization I affirm that I am the legal next of kin, property owner, or have obtained appropriate permission to authorize cleaning and media use of this headstone.
